First Appointment
On your first appointment, I will schedule additional time in order to take and discuss your Health History Intake Form (use the Insurance Information with Health History Intake form if you have specific insurance covering massage therapy). My goal is to provide the most appropriate treatment possible, given your current status, therapeutic goals, and any limitations that might be imposed due to injury or medical conditions. You will have the opportunity, prior to treatment, to ask questions and discuss any preferences or apprehensions. During your session, I will modify my pressure according to your preferences and responses.

Note: If you have cancer, please download the Oncology Massage Health History form rather than the previous general intake form. In addition, if you are receiving treatment currently, you will need to ask your doctor to read and sign the Physician Acknowledgment form.
When treating pain conditions, I will include postural analysis and orthopedic assessment prior to the manual treatment of soft tissue. Before you leave, we may review home-care suggestions, such as specific stretches or releases that you may be able to practice for continued benefit.

Insurance
Direct billing is provided for clients covered by Worker’s Compensation claims, Personal Liability/Motor Vehicle Accident claims, and City of Portland employees (or others) eligible for Anthem’s massage therapy benefit.
Cancellations & Late Arrivals
For each appointment, I have scheduled your session time along with time for pre-session assessment and post-session evaluation. If you need to cancel, please provide a minimum of 24 hours notice to avoid being asked for reimbursement for my time. For late schedule changes, call 899 - 8142, as I do not receive email in my office. I will provide reminder calls or email prior to your appointment, upon request. If you are running late, I appreciate receiving a call to let me know. I will attempt to provide your full treatment session but may not be able to in all cases.

Medical Spending Accounts
You may submit the entire cost of therapeutic massage for reimbursement from your “flex” account (some plans require physician approval, but many do not). If your insurance covers massage therapy, you may submit your co-pay to a medical spending account. Please request a receipt if you plan to do this.
